Colorado Access Partners with Innovaccer to Enhance Population Health and Quality Reporting for Medicaid Members

Colorado Access, a nonprofit health plan serving the state for over 30 years, has announced a strategic partnership with Innovaccer Inc., a healthcare AI company. The collaboration aims to leverage Innovaccer’s population health management platform to streamline reporting and enhance care coordination, with a specific focus on implementing HEDIS quality measures by June 2025.

Colorado Access serves as the largest public sector health plan in Colorado, connecting more than 530,000 Medicaid members to physical and behavioral healthcare through Health First Colorado (Colorado’s Medicaid program) and Child Health Plan Plus (CHP+). The organization is committed to improving health outcomes and supporting communities to ensure equitable, accessible care.

Innovaccer’s Data and Analytics platform is expected to enhance care management and improve quality outcomes for Colorado Access. By integrating Innovaccer’s Data Platform, Quality Management & Reporting, Population Health Analytics, and Provider Engagement tools, Colorado Access will gain real-time visibility into quality metrics, streamline risk assessments, and drive proactive interventions to improve member outcomes across its network.
